The Smart
Care Project.

The SMART CARE project aims to create a national network for the management of patients with chronic heart failure by developing and validating an Integrated Monitoring System that optimizes prevention, diagnosis, treatment, and follow-up, improving the effectiveness of the health care system and reducing improper examinations, visits, and hospitalizations.
