The SMART CARE project, funded by the Ministry of Health under the Health Operational Plan – Trajectory 2 “eHealth, advanced diagnostics, medical devices and mini-invasiveness,” Line of Action 2.1 “Creation of a national network for high-impact diseases,” has as its main objective the creation of a national network for the management of the patient with chronic heart failure.
This will be achieved through the design, development and validation – from a technological, clinical and integration with the national health care system perspective – of an Integrated Monitoring System based on:

1. advanced solutions for home monitoring and assessment of the patient using smart, minimally invasive and wearable sensory devices;
2. Multimarkers Score for decision-making support, based on data collected by the developed technological devices;
3. Platform of telemedicine services for the Italian heart failure network.

The goal of the Monitoring System is to make the different phases of patient management (prevention, diagnosis, treatment, and follow-up) more efficient, while improving the effectiveness of the national health care system. This will be achieved by reducing the use of unnecessary tests, visits, and hospitalizations, promoting early recognition of the disease, ensuring adherence of treatments to guidelines, and promoting integrated follow-up between specialist centers and general medicine.
Finally, the project includes the generation of open data, which will be made available to the scientific and medical community to foster research, knowledge sharing, and the development of new innovative solutions in the field of health.
